    GS Slate Users/Security Form

    Thank you for your interest in partnering with Graduate Admissions to utilize Slate data.

    Please complete the form below with as much information and detail as possible to request access to Slate. Upon submission, a member of the Graduate Admissions team will follow up with you to discuss the submission and next steps. 

    You can add up to 20 programs for access on one form. PLEASE DO NOT SUBMIT MULTIPLE FORMS AT ONE TIME WITH ONLY ONE PROGRAM LISTED.

    All Slate users must agree to the Slate User Confidentiality & Security Agreement below and be up to date on their annual FERPA Training. Users will be required to complete this Agreement on an annual basis in order to continue their access.


    The purpose of this Agreement is to:
    • provide users with the terms and conditions of accessing our graduate admission system, Slate by Technolutions
    • provide our office with the necessary information to assign users the appropriate Slate role and permissions
    • maintain a current, accurate record of all Slate users

    Confidentiality Agreement
    As an approved Slate User, I agree to the following terms and conditions:
    The Slate system and related information systems are committed to the security and privacy of data in order to keep the trust of the Clemson University community. When security or privacy is compromised, confidence in the system is jeopardized and legal liability may be created for the University and for the employee responsible for the compromise. By signing this form, I acknowledge that I understand the following:

    SECURITY & PRIVACY
    1. Slate and related information systems cannot effectively serve the University when any employee loses the trust of those we serve.
    2. Your access to Slate and related information systems may have privileges to material and information that is designated as confidential by State or Federal law. Access to this material is not authorization to disclose it.
    3. Unauthorized release or sharing of confidential information from files, electronic data, notes, conversations, personal observations or knowledge as a result of your access or participation in Slate and related information systems will be referred to Clemson University Human Resources for disciplinary evaluation, which could result in discipline against me, up to and including immediate termination and/or legal action.
    4. Everyone utilizing/accessing Slate data shares the responsibility for the security, integrity, and confidentiality of information. Likewise, individuals are responsible for maintaining their FERPA training and acknowledgements annually.
    5. Everyone with privileged access to Slate and related information systems is required to sign this Confidentiality Statement acknowledging responsibility.
